Comprehensive Primary Care Plus (CPC+) quality measures for 2017

CMS recently announced the re-opening of CPC+ program applications in early 2017. For those practices who were recently accepted into the program and those who will be applying, here’s a quick-look resource on the CPC+ quality measures.

Read about other program requirements here.

The CPC+ program includes 14 eCQMs, which is four fewer than the original 18 eCQMs in the Request for Applications. All CPC+ practices will be required to report eCQMs annually at the practice site level. The performance results will determine eligibility for the CPC+ performance based incentive payment.

2017 eCQM reporting at-a-glance:

  • Performance period: January 1 – December 31, 2017
  • First eCQM submission period: Begins January 1, 2018 to CMS
